SIUE School of Pharmacy Online Application Instructions

The SIUE School of Pharmacy professional program is not a program into which freshmen can be admitted. You must have completed the following courses at a regionally accredited institution of post-secondary education to be considered for admission. Completion of the pre-pharmacy curriculum does not guarantee admission into the School of Pharmacy. All elements of your application file must be received by SIUE no later than December 1, 2012.

If you are admitted, a drug test and background check will be completed before your placement at experiential learning sites.

Application and Admission Review Process

  1. Complete applications are forwarded to the School of Pharmacy Admissions Committee.
  2. The most qualified candidates are invited to SIUE for personal interviews and to complete the writing assessment.
  3. Students will be notified of Admissions Committee decisions.
  4. Students who are admitted to the SIUE School of Pharmacy will be required to submit a $300 non-refundable enrollment deposit. This deposit will be applied toward the first semester’s tuition.


To Apply to the Pharmacy Program:

If you have completed the pre-pharmacy curriculum or if you will have completed it before fall 2012, please proceed through the following steps to complete your application.

Step 1. SIUE School of Pharmacy online application and $40 application fee (Deadline: December 1, 2012)

When you begin to complete this element of your application, you will be asked to create a user ID and PIN. It is essential that you remember your ID and PIN so that you can periodically check the status of your application. If you do not have this information, it will be impossible to check the status of your application online. Your application will not be processed unless you click the final submission button. A $40 application fee must be submitted at the time of application. An 80-cent service fee will be charged to your credit card for online payments.

Step 2. Three Grade Point Average (GPA) Calculation Worksheets

Using your own copies of your post-secondary transcripts, complete the three GPA Calculation Forms. The SIUE School of Pharmacy requires a minimum GPA of 2.5 in each of three separate GPA calculations. The cumulative GPA calculation (1) includes all undergraduate grades earned. The pre-pharmacy GPA calculation (2) includes only those courses (or equivalent courses) listed on the SIUE pre-pharmacy GPA calculation worksheet. The pre-pharmacy science and mathematics GPA (3) includes the science and mathematics courses listed on the SIUE pre-pharmacy science and mathematics worksheet. You must complete all three GPA Calculation Forms.

The GPA Calculation Forms are designed to calculate your GPA as you populate the cells. For each course you have completed, include the following: the exact course title and number, the college/university where you took the course, the term during which you took the course (e.g. spring 2012, fall 2012), the semester hours you earned for the course, and the grade you earned. Do not use (+) and (-) grades (e.g. "A-" should be listed as "A"). If you have not completed some courses in the pre-pharmacy curriculum, list them on the form as well and indicate when you plan to take the courses, which university or college you plan to take them at, and the exact course names and numbers. Do not fill in the hours column that corresponds with the courses you have not yet completed. If you make an error as you are completing the forms do not cut and paste to correct your error. Use the backspace key to return to the cell in which you made the error and reenter the information.

If you have earned Advanced Placement or proficiency credit for a pre-pharmacy course, then enter "P" in the corresponding grade cell for the course and leave the credit hours and points columns blank.

If you repeat a course in the Pre-Pharmacy curriculum, the grade from the most recent attempt should be used in the calculation of the GPA.

It is important that your documentation of courses and calculation of the GPA is correct. The Admissions Committee will check your worksheets for accuracy. If you have questions about how to complete these forms, then call the SIUE School of Pharmacy Office of Student Affairs at (618) 650-5150.

Save the three completed worksheets to your computer. Then, email the worksheets as an attachment to pharmdocs@siue.edu. In the subject line, type your name. You need not provide any information in the body of the email.

Step 3. Two Evaluation Forms (Deadline: December 1, 2012)

Provide Two Evaluations. Print two copies of the evaluation form. Complete the top portion of each evaluation form and give each evaluator a form and a stamped envelope addressed to you. Ask each evaluator to sign his or her name across the flap of the envelope prior to returning it to you. Forms completed by SIUE School of Pharmacy faculty or your relatives are not acceptable.

Step 4. Official Transcripts from all post-secondary institutions attended

You must have official transcripts mailed directly to Admissions by each post-secondary institution previously attended, except Southern Illinois University Edwardsville. Hand carried or faxed documents are not acceptable. Transcripts are considered official when mailed directly by the granting institution to: School of Pharmacy Admissions, c/o SIUE Service Center, Box 1080, Southern Illinois University Edwardsville, Edwardsville, IL 62026-1080. Deadline = December 1, 2012.

Send official transcripts at the time you apply. If you are enrolled in any courses during the fall 2012 semester, send another transcript at the end of the fall 2012 semester. Transcripts for fall 2012 courses must be postmarked by February 1, 2013.

Step 5. Official PCAT scores (October 17, 2012, is the latest test date that will meet the application deadline)

Have your PCAT scores sent directly to SIUE, PCAT code 903.
